Completing Your NEAP Lump Sum Application

Do you have questions about completing your NEAP Lump Sum Withdrawal Application? Please follow the instructions below to complete your application in its entirety. We’ve highlighted the most common mishaps to help you successfully submit a completed application

First, Page 3 details the documents you can use to prove your age.

On Page 4 indicate whether you are single, married, or divorced. You must select one of these items. Also, if you are or have been divorced, list your previous spouses.

Go to Page 5 to provide the name of your present or most recent NEAP employer, your home local and the last day you worked for this employer.

Next on Page 6, circle any of options A-D. You must choose at least one of these options.

On Page 7, your spouse must complete the Spouse Option form if your account balance exceeds $5,000.

Turn to pages 8 and 9. If you plan to rollover your NEAP benefit into an IRA or defined contribution plan, your financial institution must complete the IRA or the defined contribution rollover authorization. Have your plan official sign the form including their name and title.

Finally, on page 10 remember to sign your completed NEAP application.

Page 11 is another look at the documents we need to expedite the application process. Once you complete your application please thoroughly review it before submitting it to our office.
